Sec. 1. On or before March 1 of each year, a health maintenance organization must file with the commissioner a report that covers the preceding calendar year. The report must be: (1) made on forms prescribed by the commissioner; and (2) verified by at least two (2) principal officers of the health maintenance organization. As added by P.L.26-1994, SEC.25.
