( Chief Executive) Name and location of bank. We, the undersigned directors of __________ bank, have reviewed the report of the Commissioner of Banking under date of __________. Effect of Amendment 2021 Act No. 30, SECTION 4, substituted "Commissioner of Banking" for "State Board of Bank Control" in three places, and substituted "Chief Executive" for "President or Cashier".
